1、In the past, operations were difficult. Until the middle of the eighteen fifties, surgery was very dangerous. Many patients died after even the smallest operations. This was because bacteria entered the cuts in the patients' bodies and started infection. In some countries, up to 90 percent of patients died from infection after operations. In 1865, however, Joseph Lister, a British surgeon, found an answer to the problem. He used an "antiseptic"(消毒剂)during and after operations. This killed the dangerous bacteria and most of his patients lived. Since then, surgeons have used antiseptics in all operations.

步行是所有日常生活活动中最基本的动作。因此在早期康复治疗的目标和训练计划中都应充分地考虑步行所必须具备的功能,并予以认真地指导与实施,为今后进行步行治疗移动训练奠定坚实的基础。协调性、可动性和稳定性是步行的三要素。正常步行必须具备支撑体重,保持平衡和迈步的能力。步行中所含的动作(足跟着地、单腿支撑、足跟离地、摆动等) 都要求身体各部位的协调运动,在步行中形成一个完整、精细、熟练的、连续过程。丧失步行能力的患者,因疾病、造成障碍的原因、存在的问题和康复的目标不同,具体训练方案有较大差异。康复治疗人员应在正确分析患者存在的问题的基础上,制定出个性化训练方案。本讲就偏瘫步态的障碍特点提出训练的基本思路和实施要点。 偏瘫步态训练包括站立相负重训练、重心移动训练、行走训练、日常生活环境下的步行训练。 一、站立训练(平衡训练〉 步态训练通常是从患腿的站立训练开始。从平衡功能检测仪中可以消楚地显示出人体重心摆动轨迹。由于站立立支持面小重心位置高,

任何体内、外的轻微变化都会对全身姿势产生很大影响。为了维持站立,肌肉必须不停地进行收缩活动。

站立时,重心的摆动与伸肌活动,特别是小腿三头肌的活动有着密切的关系。在临床中不少的患者既使站立困难,也可以步行。但是应该注意的是,这种步行由于缺乏基本条件,患者往往形成异常步态,造成错误的动作习惯,增加矫正的难度; 并且不少患者出现关节及肌肉损伤导致下肢关节的疼痛;更重要的是由于步行的稳定性差很容易跌倒造成事故,因此对于步行训练,不可急于求成。要将基本的功能训练与应用动作训练有机的结合、严格按照训练计划进行才可能收到良好效果。 (一)诱发平衡控制模式训练 某些患者由于不能在平衡受到威胁时做出相应的动作反应而使身体的稳定性下降,身体晃动增加甚至摔倒。前庭功能丧失者常常不能诱发出髋关节协同运动,因此,躯体感觉输入丧失者则无法选择踝关节协同运动。因此,对于此类患者,运用和掌握正确的自动姿势协同模式是治疗的重点。多余的、不必要的或代偿性的动作或运动必须加以抑制,最终使肌群活动能够根据平衡功能活动的需要采取正确地组合与编排。

常见中枢性运动控制障碍异常步态的分析与训练步行训练是患者和家属最关心的项目之一,患者也常因疾病的影响或急切期待提高步行能力,而忽视了基础训练,诱发并强化了反向负荷动作,形成了各种异常步态。中枢性控制障碍患者的康复治疗目标之一就是在现有功能障碍基础上帮助患者实现较高水平的功能独立,步行是功能独立的基本要素之一。治疗师在制定训练方案之前应实施步态分析,针对患者的步行姿态、步行效率、关节及肌肉活动、平衡协调控制等多方面因素进行检查,以确定异常步态的特征及原因,以帮助确定治疗方案及选择辅助具。 一、常用的步态分析方法 最常用的步态分析方法为临床观察法、录像观察、足印法和三维运动分析法。前三种经济实用,在临床上使用较为普遍;三维运动分析法是最为精确的步态分析方法,包括时间/空间参数测定、压力测定、表面肌电、气体代谢等方式,能够反映步态的运动学、动力学,相关肌肉活动及能量消耗的情况,需要运动捕捉系统、测力平台等器械,国际上已广泛使用。 二、步态参数 1步行周期是指一足着地到同侧足再次着地的过程,包括距离和时间参数。在正常步态中,步行周期开始于一侧足跟着地,结束于同侧足跟再次着地;在某些异常步态时,足跟不一定是足和地面首先接触的部位,那步态周期就可看成是一足某个部位接触地面到同侧足再次接触地面的过程。步行周期可分为支撑相和摆动相两个相。 (1)支撑相下肢接触地面和承受重力的时相,占步行周期的60%,包括: ①早期(early stance) 包括首次触地(initial contact)和承重反应(loading response),正常步速时占步行周期的10%~12%。其中双支撑相为支撑 足首次触地及承重反应期相当于对侧足的减重反应和足离地,由于此时 双足均在地面,又称之为双支撑相。双支撑相是步行周期中最稳定的时 期。②中期(mid stance) 即单支撑相,支撑足全部着地,对侧足处于摆 动相,是唯一单足支撑全部重力的时相,正常步速时大约为步行周期的 38%~40%。③末期(terminal stance)指下肢主动加速蹬离(push off)的 阶段,开始于足跟抬起,结束于足离地约为步行周期的10%~12%。(2)摆动相下肢在空中向前摆动的时相,占步行周期的40%,包括:①早期(initial swing) 主要的动作为足廓清地面和屈髋带动屈膝,加速肢体 前向摆动,占步行周期的13%~15%。②中期(mid swing) 足廓清仍然 是主要任务,占步行周期的10%。③末期(terminal swing) 主要任务 是下肢前向运动减速,准备足着地的姿势,占步行周期的15%。 2 步长(Step length) 也称步幅,指一足着地至对侧足着地的平均距离,可分为左侧步长和右侧步长,在异常步态中,两者可能差距很大。 3跨步长(stride length)由左侧步长和右侧步长组成,相当于一个步行周期的距离。
