抗凝剂对体外诊断试剂检测结果的影响及其干扰研究方法

抗凝剂常用于临床检验中对人全血样本或血浆样本的处理。不同种类抗凝剂的结构、性质差异较大，且作用机制不同，其选择则需根据临床检验中的具体项目和相应使用的检测试剂来定。不同的抗凝剂会影响血液不同的成分，或干扰检测试剂的主要成分，从而影响最终的检测结果。因此，一方面为提高体外诊断试剂在临床检测项目中结果的可靠性，另一方面在体外诊断试剂注册审评过程抗凝剂的干扰研究作为较为关键的环节，体外诊断试剂申请人应结合整个检测系统来严格评估抗凝剂的适用性。应当在体外诊断试剂研发及临床试验过程，根据产品的实际使用情况对抗凝剂进行干扰研究，并将干扰研究结果作为一项注册申报资料提交。下面将结合抗凝剂作用机制、对体外诊断试剂检测结果造成影响的因素等，提出注册审评过程对抗凝剂的关注点和干扰研究方法，供体外诊断试剂申请人参考。
1.常用抗凝剂的作用机制
临床检测项目中常用抗凝剂的作用机制主要分为两种：一是抗凝剂中相应结构会与抗凝血酶因子结合，提高抗凝血酶因子的活性，加速对凝血酶因子的灭活，从而抑制凝血酶原激酶的形成等达到抗凝的目的，如肝素等；二是与

内容提要：当体外诊断试剂预期用途明确，确定检测的临床样本类型为血浆或全血时，申请人在产品说明书中应同时明确样本添加的抗凝剂类型，并提供抗凝剂对检测结果是否产生干扰的研究性资料。该文主要基于临床样本常
用抗凝剂的作用机制，和对体外诊断试剂使用过程的影响因素等，并结合《体外诊断试剂注册管理办法》等
相关法规的要求[1]，来探讨抗凝剂在外诊断试剂的研发阶段及临床试验过程中的干扰研究方法。

Influences of Anticoagulant on Detecting Results of IVD Use and Its Interference Studying Methods
