影响微种植体支抗初期稳定性的因素分析

[摘要]目的:探讨影响微种植体支抗初期稳定性的因素,以提高正畸疗效。方法:选择2014年3月-2015年3月来笔者科室就诊的60例错牙合畸形患者为研究对象,共植入微型种植钉160枚,对所有患者进行负载期10个月观察,统计患者微型种植钉的成功率及微型种植钉支抗初期稳定性的影响因素。结果:60例患者中,微型种植钉成功54例(占90.0%)。不同年龄、性别、微型种植钉长度、加力状况,患者微型种植钉成功率对比无显著性差异(P >0.05)。不同下颌平面角、口腔卫生状况、肿胀状况、牙周炎史患者微型种植钉成功率对比,有显著性差异(P <0.05)。结论:下颌平面角、口腔卫生状况、肿胀状况、牙周炎史等是微种植体支抗初期稳定性的影响因素,临床需加强对影响因素控制,积极防治患者口腔肿胀,保证患者口腔卫生,以提高正畸疗效。

Analysis of the factors affecting the primary stability of micro-implant anchorage

Abstract: Objective The purpose of this study was to evaluate the factors affecting the primary stability of micro-implant anchorage, in order to improve the effect of orthodontic treatment. Methods From March 2014 to March 2015,60 cases of patients with malocclusion in our hospital were chosen as the object of this study, and they were implanted by 160 micro-implants. All patients were observed in 10 months of load period, and then the successful rates of micro-implant anchorage and the factors affecting the primary stability of micro-implant were compiled. Results Among 60 cases of patients with micro-implants, they were successful in 54 cases which accounted for 90.0%. There were no significant differences ( P >0.05) in the successful rates of micro-implants with different age,gender,length of micro-implant, load condition. However, the successful rates of micro-implants were statistically significant ( P <0.05) in patients with different angle of mandibular plane, oral hygiene status, swelling condition, history of periodontitis. Conclusion Angle of mandibular plane, oral hygiene status, swelling condition, history of periodontitis were the factors affecting the primary stability of micro-implant anchorage. In order to ensure the effect of orthodontic treatment, the factors affecting the primary stability of micro-implant anchorage should be under more control with active prevention and treatment of patients with swelling in the mouth and good oral hygiene of the patients. 中支持移动矫正牙体引起的反作用力[1]。微种植体良好的支抗效果及操作简单等方面的特点使得其在临床上的运用越来越多,但随着临床运用的广泛性,其支抗初期的稳定性也成为临床关注的重点。此次研究基于上述背景,探讨微种植体支抗初期稳定性的影响因素,以期提高微型种植钉的成功率,现报道如下。

1 资料和方法

1.1 一般资料:选择60例错牙合畸形患者作为研究对象,均符合正畸治疗相关诊断标准。其中男27例,女33例;年龄20~38岁,平均年龄为(26.9±4.7)岁;共植入微型种植钉160枚:前移后牙42枚,增强矢状向支抗内收前牙80枚,齿科美容
